Verification of Registration/ Enrolment
Nurse/Midwife can apply for verification of registration/enrolment with SNB to prove his/her current SNB registration status to another nursing regulatory body/registering authority.
Application for verification of registration/enrolment with SNB must be made by the individual nurse/midwife.
How to apply for verification of registration/enrolment /certificate of good standing (CGS)

For applicants in Singapore with Singpass account
Login to make online application for verification of registration/enrolment. Please refer here for login instructions and instruction to reset your password.Upon successful login, select “Application” from left hand menu and click on “CGS/COPS”.An Application for Verification of Registration (VoR) will be launched. Complete all required information.
Note:
i) Fields marked with * are mandatory.
ii) Click on “Add Requesting Authority” then select “Self-Collection” for the Send By option and enter “N.A’’ as the Reason for Self-Collection so that you can proceed on with the application. SNB will send the verification of registration/enrolment directly to the requesting authority. Hence, please ensure that you entered the name and the address of the requesting authority correctly. Incorrect information may result in non-receipt of the verification of registration/enrolment by the requesting authority. In this case, you will be required to make a new application (including payment).
iii) You may add more than one requesting authority in the same application. However, the system will provide one application number for each requesting authority. A fee is required for each requesting authority.
Documents required to be uploaded under the “Additional Documents’’ field in your online application:
a) A certified true copy of your original SNB ‘Certificate of Registration/Enrolment’ (B5 size certificate with SNB’s embossed seal, not the practising certificate – small blue card) is required.
*Note: If you have lost/misplaced your SNB Certificate of Registration/Enrolment, you must apply and pay for a replacement of it. Refer here for more information on replacement of your SNB Certificate of Registration/Enrolment.
b) Duly completed and signed Requesting Authority’s Verification of Registration/License form (if applicable). (SNB may require the original hard copy to be sent to us if the scanned copy is not clear).
Note:
i) Documents that are not in English must be accompanied by an accurate translation in English and certified by Justice of Peace, Notary Public from their country of origin.
ii) Documents certified as “true copies”
– Must be initialled on every page of the document by the authorized officer: Justice of Peace, Notary Public or your employer (applicable to Singapore employers only), unless otherwise stated.
– Must include the name of authorized officer (full name), signature, date of certification and institution’s stamp/seal and the notarization must be made in English.
iii) Photocopies of previously certified documents will not be accepted.
iv) Copies which are not acceptable to SNB will result in the delay of processing of your application.The SNB Fee for Application for Verification of Registration/Enrolment (inclusive of registered mailing fee) is payable before submission of this application.
Payment must be made by:
i) eNets Credit/Debit Card
>> Acceptable Credit Cards (VISA and MasterCard ONLY) [Note: American Express (Amex), Diners Club and JCB Credit Cards are not accepted.]
>> Acceptable Debit Cards (VISA Electron Debit Cards and MasterCard ONLY)
ii) eNETS Internet Banking
>> Participating Banks (DBS, POSB, UOB, OCBC, Citibank ONLY)
Your application will only be processed when all the required documents are submitted. Incomplete information and/or failure to submit required documents will result in the delay of processing of your application.
The average processing time for verification of registration/enrolment is 14 working days from the date of receipt of your application and its fee and this excludes mailing duration. Your request will be sent directly to the nursing regulatory body/registering authority (as provided in your Application) by registered mail (default mode of postage). Estimated mailing duration via registered mail is 10-15 working days.If you require your verification of registration/enrolment to be sent via DHL courier (subjected to additional costs), please email us at SNB@spb.gov.sg to confirm the actual cost (depending on the country of requesting regulatory authority. Your email must include your application number, full name as in Practising Certificate and contact details. Estimated mailing duration by DHL courier is 3-5 working days. *You are advised to opt for DHL courier to expedite the mailing process if the verification of registration/enrolment is required urgently as expedition of applications will only be considered on a case by case basis and is solely at the discretion of SNB.
For applicants overseas without Singpass account
Note: This section is only applicable to nurses / midwives who do not have Singpass and are not eligible for One-Key Token.
- Download and complete the Application for Verification of Registration/Enrolment Form.
Note: If you require verification of registration/enrolment to be sent to more than one requesting authority, you will be required to complete a new application form for each additional requesting authority. - Documents required:
a) A certified true copy of your original SNB ‘Certificate of Registration/Enrolment’ (B5 size certificate with SNB’s embossed seal, not the practising certificate – small blue card) is required.
*Note: If you have lost/misplaced your SNB Certificate of Registration/Enrolment, you must submit a certified true copy of your current passport (photograph page) as an alternative form of identification.b) Original duly completed and signed Requesting Authority’s Verification of Registration/License form (if applicable).
Note:
i) Documents that are not in English must be accompanied by an accurate translation in English and certified by Justice of Peace, Notary Public, Commissioner of Oath from their country of origin.
ii) Documents certified as “true copies”

– Must include the name of authorized officer (full name), signature, date of certification and institution’s stamp/seal and the notarization must be made in English.

iv) Copies which are not acceptable to SNB will result in the delay of processing of your application. - The SNB Fee for Application for Verification of Registration / Enrolment (inclusive of registered mailing fee) is payable upon submission of this application.
Available payment modes:
a) PayNow to Singapore Nursing Board using our Unique Entity Number (UEN) number: T08GB0055E (only applicable to Singapore bank accounts). When making payment, please ensure the following details are indicated:
– Applicant’s name (as it appears on SNB practising certificate)
– Applicant’s SNB Registration/Enrolment Number
– Fee Type: CGS Application
Once payment is made, please email to SNB@spb.gov.sg with a screenshot of the payment transaction to facilitate for payment verification. Payment verification will take 3 to 5 working days and you will be informed should payment be unsuccessful.
b) Authorisation of a proxy for payment by NETS at SNB’s office (strictly by appointment only, email to SNB@spb.gov.sg for appointment).
Note: If you require your verification of registration to be sent via DHL courier (subjected to additional costs), please email us at SNB@spb.gov.sg in advance to confirm the actual cost (depending on the country of requesting regulatory authority). Your email must include your full name as in Practising Certificate and contact details. The advised courier fee must be included in the PayNow transaction amount. - The average processing time for verification of registration/enrolment is 14 working days from the date of receipt of your application and its fee and this excludes mailing duration. Your request will be sent directly to the nursing regulatory body/registering authority (as provided in your Application) by registered mail (default mode of postage) or DHL courier (as per your request, if applicable) Estimated mailing duration is 10-15 working days for registered mail and 3-5 working days for DHL courier. *You are advised to opt for DHL courier to expedite the mailing process if the verification of registration/enrolment is required urgently as expedition of applications will only be considered on a case by case basis and is solely at the discretion of SNB.

Note: If you are authorising a proxy to submit the application and payment on your behalf at SNB’s office, please email to SNB@spb.gov.sg for an appointment for your proxy. A duly signed authorisation letter must be submitted along with this application form. The authorised person is also required to present his/her original NRIC/Passport for SNB’s verification. SNB reserves the right to reject applications in the event that no prior appointment is made and if any of the required documents is not produced.
