How do I submit my medical report to LTA?

Medical Assessment

Vocational licence holders at the age of 50 and above must undergo regular medical check-ups every 2 years to keep their licence valid, even if their licence does not expire that year. Vocational licence holders will receive a notification letter 2 months before their licence expires, to inform them to go for their medical check-up. 

Age 50 to 64

Vocational licence holders from the age of 50 to 64 must complete a medical check-up by bringing LTA’s Medical Examination Form (PDF, 799kB) to any Singapore-registered clinic or SATA Diagnostic Centre every 2 years. The completed form should then be submitted to LTA_Medical@lta.gov.sg.

If ‘Yes’ is indicated at any of the listed conditions in Section F of the medical form, an additional memo is required from a medical specialist that certifies the driver is fit to drive a Public Service Vehicle.

Age 65 and above

Upon reaching 65 years of age, medical requirements must be met every year. Vocational drivers may need to complete additional medical assessments depending on whether they hold a Class 3 or Class 4 licence.

Drivers who are only Class 3 Drivers

Here are the steps to complete the Assessment on Fitness to Drive for Class 3 drivers turning 70, 73 and 74 years old:

1.    Bring LTA’s Assessment on Fitness to Drive Report (PDF, 206kB) to any general practitioner and do a check-up. The doctor will fill in and certify Parts I and II of the report.

2.    Take an on-road assessment at any of the three driving centres: Bukit Batok Driving Centre, ComfortDelGro Driving Centre, and Singapore Safety Driving Centre. Bring along your NRIC. After you pass the on-road assessment, the Driving Centre will endorse the Assessment on Fitness to Drive Report and will also give you a completed On-Road Assessment Checklist.

3.    Submit both the Assessment on Fitness to Drive Report and On-Road Assessment Checklist Form to LTA_Assessment@lta.gov.sg. If you are turning 74 years old, you should also submit both the Assessment on Fitness to Drive Report and On-Road Assessment Checklist Form to the Traffic Police via https://eservices.police.gov.sg

If you are driving a modified vehicle(e.g. with a spinner knob, left foot accelerator), please take your on-road assessment with Tan Tock Seng Hospital’s Occupational Therapy Clinic.

If you are a taxi driver or private hire car driver, the driving centre will provide a car for the on-road assessment. If you are a Class 3 bus driver, you need to use your own bus to take the on-road assessment.  If you have vocational licences to drive both types of vehicles, you have to take the on-road assessment twice, once with a car and once with a bus. You need not submit your report until you have passed your on-road assessment.

Drivers who are only Class 4 Drivers, or who are both Class 3 and Class 4 Drivers

If you are a Class 4 vocational driver, or you are both a Class 3 and Class 4 vocational driver, you will need to:

·       Complete Traffic Police’s Enhanced Medical Examination form and Enhanced Proficiency Driving Test Report

Submit the completed form and report to LTA at LTA_Assessment@lta.gov.sg and to Traffic Police at https://eservices.police.gov.sg.
